She probably keeps a backup copy of Miles in the buffer just in case.
Why would she? Keiko has repeatedly shown she absolutely detests Miles. That marriage is colder than Siberia in a harsh winter. The sheer amount of passive aggressive hatred that oozes out of her every time they’re on screen is remarkable.
Keiko wishes every day that Miles will finally die, and resents him for never doing so.
Kirayoshi wasn’t Obrien’s son. Keiko was on bajor for months, admittedly had a close emotional relationship with a bajoran man on her team, and the baby had to be transplanted into a bajoran woman in order to survive gestation.
This is not only my head cannon, I believe it was the writers intent.
Honestly, that episode where she’s possessed by the pah wraith is the least cold and awful we ever see her…
she allowed a PAH-WRAITH to possess her again. im surprised she dint try to kill him the first time being posesses by a pah-wraith.
an evil miles variant died on another planet in the pegasus galaxy from a nuke.
Miles should just show up in Starfleet Academy, just fixing things and saving the day, but with no explanation given as to why he is still alive.
He’s on a first name basis with the Koala.

You’d think he’d have jury rigged a turbo lift to the top by now.
They’ll have to wait 'til next Tuesday to install the backup Miles.
In a crunch, I wouldn’t like to be caught without a second backup Miles.
O’Brien will never be able to escape his life of torment. Even death won’t put an end to it.
